Job summary

Oakland Community Health Network seeks a Deputy Director of Integrated Care to lead and oversee healthcare integration across the network. You will shape strategies, coordinate contracts, grants, and quality measures while guiding staff and partners to deliver high-quality, person-centered services.

The role requires clinical leadership experience, strong data-driven decision making, and collaboration across departments.

Qualifications

• Minimum five years of relevant clinical leadership experience.
• Leadership experience in a managerial or supervisory role in the public or private sector is preferred.
• Departmental leadership or director-level experience is required.

Responsibilities

• Lead healthcare integration initiatives that support organizational, contract, and strategic objectives.
• Provide supervision, coaching, performance management, and professional development for the Manager of Integrated Healthcare and assigned integrated care staff.
• Lead and represent OCHN in healthcare integration committees, workgroups, collaborative partnerships, and cross-system initiatives.
• Negotiate, manage, and monitor integrated healthcare contracts, grants, related funding, deliverables, and performance expectations.
• Develop, implement, and evaluate integrated care protocols, practice standards, workflows, and quality improvement activities.
• Collaborate with providers and internal and external stakeholders to support integrated care training, workforce development, and practice improvement efforts.
• Evaluate provider performance and contract compliance for integrated healthcare initiatives and recommend corrective actions or recognition based on outcomes and quality measures.
• Analyze clinical, quality, utilization, and integrated healthcare data to identify trends, improve performance, and support strategic decision-making.
• Resolve provider and system-level issues related to integrated care, including complaints, disputes, performance concerns, and barriers to coordination.
• Collaborate across departments and with external partners on local, state, and federal initiatives impacting integrated physical and behavioral healthcare.
• Develop and monitor assigned budgets, grant expenditures, and resources for integrated healthcare initiatives.
• Perform other duties as assigned.
