Job summary

Mercy Primary Care in Chesterfield is seeking a Primary Care Nurse to collaborate with Providers and the care team to deliver safe, timely, patient-centered care. You will handle triage calls, support clinic operations, and help drive quality outcomes for an attributed panel.

Initial on-site training is required before transitioning to a 50/50 hybrid work model. Schedule is Mon-Fri 8:00–16:30 with no nights, weekends, or holidays.

Qualifications

• LPN Diploma, active LPN license, 2–3 years exp preferred.
• RN: AS/BSN or 4 years exp in lieu of BSN, active RN license, 2–3 years exp preferred.

Responsibilities

• Manage triage calls/messages for the clinic and support providers.
• Collaborate with Providers and care team to drive quality outcomes.
• Assist with clinic operations and transition to hybrid work after training.
