Job summary

Alliance Health is seeking a Community Health Worker for a full-time hybrid position covering Wake and Cumberland counties in North Carolina. The role focuses on engaging members, performing SDOH assessments, connecting to community resources, and supporting benefits enrollment.

Travel within Alliance communities may be required. Qualified candidates typically have a HS diploma or GED (or an associate degree in human services) and 2+ years of behavioral health experience, with CHW certification

Qualifications

• High school diploma or GED with 2+ years experience in behavioral health OR 4 years lived experience across MH/PH/SS/Jus systems.
• Associate’s degree in human services with 2+ years experience in behavioral health.
• NC Community Health Worker Certification within 12 months of hire.

Responsibilities

• Complete SDOH assessments and reassessments.
• Review and update plan of care activities.
• Assist members to access community resources and services.
• Educate members on care coordination and engagement with services.
• Provide benefits consultation and enrollment assistance.
• Maintain timely documentation of care coordination activities.
• Ensure compliance with policies and procedures.
