Job summary

Alliance Health is seeking a Community Health Worker for a full-time hybrid role serving Wake and Cumberland counties in North Carolina. The role involves engaging with community members, assessing social determinants of health, and linking them to needed services through NCCare360 and other resources.

Responsibilities include education, care coordination, and assisting with benefits enrollment. Travel within Alliance communities may be required, with on-site meetings as needed.

Qualifications

• High school diploma or GED and 2 years of experience working with individuals with behavioral health needs, OR 4 years lived experience in navigating relevant systems.
• NC Community Health Worker Certification within 12 months of hire is required.
• Preferred: WRAP; IPS-SE; WHAM; Trauma Informed Care; MH First Aid; Community Inclusion/Integration; Harm Reduction; Recovery Model.

Responsibilities

• Complete SDOH assessments and reassessments.
• Provide face-to-face and field/community-based support to members.
• Submit referrals and track outcomes in NCCARE360 Platform.
• Assist with Medicaid enrollment and address issues with DSS as needed.
