Maryland Care Management, Inc. (MCMI) seeks a Community Health Worker to provide short-term care coordination in clinical and community settings. You will motivate patients, help them engage in their health and well-being, and coordinate with medical providers and social services.

The role includes home-based support, documentation, education on health services, and assistance with social determinants of health. Hybrid work with travel to Cumberland, MD is required.

Formación

• High school diploma or GED required.
• Minimum 2 years serving Medicaid populations, MD/DC preferred.
• Community Health Worker certification preferred.
• Experience in care management is preferred.
• Emergency Department experience is a bonus.
• Ability to coordinate with social services and community resources.

Responsabilidades

• Assist members in homes, community, or clinic; explain program purpose and impact on well-being.
• Document encounters and contracts; submit monthly reports; maintain electronic files.
• Educate on proper Emergency Room use and alternatives; coach on chronic condition management.
• Assist members in accessing medical home and overcoming barriers to care.
• Provide advocacy during initial medical visits to ensure needs and referrals are conveyed.
• Expand knowledge of community resources and facilitate access to housing, food, and services.
