Job summary

Maryland Care Management, Inc. (MCMI) is seeking a Care Coordinator to support the CM team in providing short-term care coordination and connection to resources for program members.

You will collect Health Risk Assessment data, screen for eligibility, refer members to services, and coordinate transitions of care with PCPs and providers, while documenting interactions and promoting member engagement.

Qualifications

• Strong interpersonal, communication, and customer service skills.
• Knowledge of medical terminology helpful.
• Knowledge of transportation and other barriers to care.

Responsibilities

• Provide administrative support to the CM team.
• Collect data for Health Risk Assessments (HRA).
• Screen for eligibility and benefits.
• Identify members without a PCP and refer to Member Services.
• Screen members by priority for CM services and refer as appropriate.
• Perform transition of care duties, including contacting PCPs or other providers for information.
• Document encounters and contacts; submit monthly reports; maintain electronic member records.
• Motivate members to be active in their health and well-being.
• Assist members in arranging PCP visits and care coordination under the Care Manager.
• Assist members in accessing health-related services and social resources.
• Verify appointments and obtain medical records as needed.
• Facilitate access to community resources (housing, food, prenatal classes, etc.).
• Work collaboratively within the team and build relationships with members and providers.
