Job summary

UNC Health in Morrisville, NC is seeking a Community Health Outreach professional to advance access to care through culturally responsive engagement, language access support, and patient navigation.

You will build partnerships with community groups, assist with vaccination clinics and mobile health events, and document outcomes in the EPIC system. This hybrid role requires a bachelor’s degree, a valid driver’s license, and at least three years in related fields; Spanish bilingual preferred.

Qualifications

• Bachelor’s degree from four-year college or university preferably in a related field.
• Requires valid driver's license.
• Minimum of three (3) years of experience in community outreach, community health, public health, language access services, program administrative support, or related field.
• Demonstrated computer skills with Microsoft Office, PowerPoint, word processing and database applications.

Responsibilities

• Supports community outreach, including mobile health clinics and community events; plans, coordinates and executes outreach activities.
• Supports community-based clinics, CHE Mobile Health clinics, and telehealth visits, including interpretation services and EPIC documentation.
• Provides navigation support to connect patients to health services and social determinants of health resources.
• Maintains current knowledge of health trends and UNC Health programs; participates in community events.
• Educates community members on health topics and provides evidence-based health education materials.
• Performs document translations and language interpretations to increase accessibility for non-English populations.
• Ensures consistent program data and activity tracking with narrative reports, presentations and graphs.
• Performs other duties as assigned.
