Qualifications

• Bachelor’s degree from four‑year college or university.
• Minimum of three (3) years of experience in community outreach, community health, public health, language access services, program administrative support, or related field.
• Requires valid driver's license.

Responsibilities

• Supports community outreach events including mobile health clinics and health fairs.
• Provides navigation to health services and social determinants of health resources.
• Coordinates documentation in the EPIC system.
• Delivers health education and translations for language access programs.
• Keeps program data and reports updated.
