Job summary

UNC Health in Morrisville, NC is seeking a Community Health Outreach Specialist to advance access to care through culturally responsive engagement, language access support, patient navigation, and health education.

The role supports CHE’s mobile clinics, telehealth, and community events, coordinating partnerships and tracking program data to improve continuity of care across North Carolina's health system. Spanish bilingual preferred.

Qualifications

• Bachelor’s degree from four-year college or university preferably in a related field.
• Requires valid driver's license.
• Minimum of three (3) years of experience in community outreach, community health, public health, language access services, program administrative support, or related field.

Responsibilities

• Supports community outreach, including mobile health clinics, community health fairs, forums, and events.
• Supports community-based and CHE Mobile Health clinics and telehealth visits, including interpretation services and EPIC data entry.
• Provides navigation to connect patients and residents to health services and social determinants of health resources.
• Maintains knowledge of health trends and UNC Health programs; participates in community events.
