Job summary

UNC Health in North Carolina is seeking a Community Health Outreach Specialist to advance access to care through culturally responsive engagement, language access support, patient navigation, resource coordination, and health education.

The role builds strategic partnerships to improve continuity of care across the statewide health system and supports vaccination clinics, biometric screenings, and data entry at outreach events and community-based clinics.

Qualifications

• Bachelor's degree from four-year college or university preferably in a related field.
• Minimum of three (3) years of experience in community outreach or related field.
• Valid driver's license.
• Proficient with Microsoft Office, PowerPoint, and database applications.
• Spanish bilingual preferred.

Responsibilities

• Plan, coordinate, and execute outreach events including mobile clinics and health fairs.
• Support community clinics and telehealth visits with documentation in EPIC.
• Provide navigation to health services and social determinants of health resources.
• Promote accessible health services and current UNC Health programs.
• Translate documents or provide language interpretation as needed.
