Job summary

Activate Care is seeking a Clinical Support Supervisor to provide clinical guidance and escalation support to Community Health Workers delivering the Utah LIFT Path Assist for Chronic Disease program. This role ensures CHWs are equipped to support patients with diabetes, hypertension, obesity, COPD, asthma, and cardiovascular risk within non-clinical CHW scope.

Qualifications

• Active RN license and clinical experience with adults with chronic conditions.
• Experience in care management, population health, or primary care.
• Ability to translate clinical concepts for non-clinical staff.

Responsibilities

• Serve as a clinical resource and provide structured case consultation for CHWs when participant needs exceed routine non-clinical care-coordination support.
• Help CHWs distinguish between coaching/navigation activities and issues requiring clinical escalation, without providing diagnosis or treatment.
• Provide subject-matter expertise on diabetes, hypertension, obesity, COPD, asthma, and cardiovascular risk.
• Support standardized CHW workflows for chronic disease education and telehealth access.
• Coach CHWs in motivational interviewing and goal-setting for patient self-management.
• Develop and reinforce clinical escalation protocols with participating FQHCs.
