Job summary

Activate Care is seeking a Clinical Support Supervisor based in Southern Utah to mentor Community Health Workers delivering the Utah LIFT Path Assist program for chronic disease management. You will provide clinical guidance while ensuring non-clinical scope boundaries and adherence to HIPAA.

You will collaborate with FQHC teams, lead escalation protocols, and train CHWs on education, telehealth, and motivational interviewing, with travel to partner clinics as needed.

Qualifications

• Current RN license or equivalent clinical licensure.
• Experience supporting adults with chronic conditions (diabetes, hypertension, obesity, COPD, asthma, etc.).
• Experience in care management, population health, primary care, chronic disease management, or similar programs.
• Ability to translate clinical concepts into guidance for non-clinical staff.
• Willingness to travel within Utah; valid driver’s license and reliable transportation.

Responsibilities

• Serve as a clinical resource and provide structured case consultation for CHWs when participant needs exceed routine non-clinical care-coordination.
• Help CHWs distinguish between coaching/navigating activities and issues needing clinical escalation, avoiding independent diagnosis or treatment.
• Provide subject-matter expertise on common chronic conditions (diabetes, hypertension, obesity, COPD, asthma, cardiovascular risk).
• Support standardized CHW workflows for education, telehealth access, appointment follow-through, and clinical escalation.
• Coach CHWs in motivational interviewing, goal-setting, and approved educational content to support self-management.
• Develop, maintain, and reinforce clinical escalation protocols with participating FQHCs, ensuring clear pathways to clinic contacts.
• Guide CHWs on when/how to escalate symptoms, medication concerns, abnormal information, or safety concerns.
• Review safety events, escalation failures, or near misses; suggest corrective actions and keep CHW boundaries with licensed care.
• Build strong relationships with FQHC clinical leads, providers, nurses, and care-management staff to align Path Assist with clinic workflows.
• Facilitate communication with clinical teams during case conferences, workflow discussions, and quality-improvement meetings.
• Deliver or support CHW training on chronic disease basics, red flags, escalation pathways, medication support boundaries, health literacy, and telehealth prep.
• Develop practical case-based training scenarios and review chronic disease education materials for clinical appropriateness.
• Support workflows for preparing patients for provider-delivered telehealth encounters and post-visit follow-ups.
• Review program trends related to escalation, appointment follow-through, telehealth completion, and chronic disease barriers to drive quality improvements.
• Collaborate with Data and Evaluation teams to review performance, safety indicators, and clinical quality measures.
• Maintain HIPAA, privacy, security, documentation, and organizational policy compliance.
• Other duties as assigned.
