Job summary

Activate Care is seeking a Clinical Support Supervisor to guide Community Health Workers delivering Utah LIFT Path Assist for chronic disease. You will support CHWs with case consultation, ensure alignment with clinic teams, and reinforce escalation protocols to protect patient safety and program quality.

Role requires RN license and ability to travel to Utah sites, with hybrid remote work options and ongoing training to maintain clinical standards and privacy compliance.

Qualifications

• Current, active RN license required or other licensure as appropriate.
• Experience supporting adults with chronic conditions (diabetes, hypertension, obesity, COPD, asthma).
• Experience in care management, population health, or primary care programs.
• Ability to travel to Utah clinic locations and provide clinical guidance remotely when needed.

Responsibilities

• Provide clinical guidance and escalation support to CHWs delivering chronic disease programs.
• Reinforce evidence-based self-management approaches and escalation protocols.
• Coordinate with FQHC clinical teams and CHWs to align workflows.
• Develop and reinforce escalation protocols with clinic contacts and care teams.
• Coach CHWs in motivational interviewing and educational content use.
• Review safety events and advocate for corrective actions while maintaining non-clinical boundaries.
