Job summary

Activate Care is seeking a Clinical Support Supervisor based in central Utah to provide clinical guidance and escalation support to Community Health Workers delivering the Utah LIFT Path Assist program for chronic disease management. This role ensures CHWs operate within the non-clinical scope while coordinating with FQHC partners.

The ideal candidate will support case consultations, reinforce evidence-based self-management, and help develop escalation protocols, training, and quality

Qualifications

• Experience providing clinical guidance to community health workers.
• Ability to distinguish between coaching and clinical escalation.
• Knowledge of chronic diseases (diabetes, hypertension, obesity, COPD, asthma, cardiovascular risk).
• Experience with telehealth workflows and clinic integration.

Responsibilities

• Serve as a clinical resource and provide structured case consultations for CHWs when issues exceed routine care coordination.
• Define and reinforce escalation protocols with FQHCs and clinic teams.
• Support CHWs in using motivational interviewing and education content to support patient self-management.
• Coordinate with FQHC partners to align Path Assist activities with clinic workflows and plans of care.
• Lead or support CHW training on chronic disease fundamentals, red flags, and telehealth preparation.
