Job summary

Activate Care is seeking a Clinical Support Supervisor to guide Community Health Workers delivering the Utah LIFT Path Assist for chronic disease programs. You will provide clinical guidance, case consultations, and escalation support while ensuring CHWs operate within non-clinical scope.

Responsibilities include coordinating with FQHC teams, training CHWs in self-management, motivational interviewing, telehealth readiness, and safety protocols.

Qualifications

• Active RN license or equivalent clinical licensure.
• Experience supporting adults with chronic conditions.
• Experience supervising or supporting CHWs or care teams is preferred.

Responsibilities

• Provide structured case consultation for CHWs when clinical questions exceed non-clinical care coordination.
• Define when to escalate to clinical care and reinforce provider recommendations.
• Offer expertise on diabetes, hypertension, obesity, COPD, asthma, and cardiovascular risks.
• Support CHW workflows for education, telehealth access, and appointment follow-through.
• Develop and reinforce clinical escalation protocols with FQHCs.
