Job summary

Indiana Professional Management Group (IPMG) seeks a Case Manager to work in a hybrid role, combining remote home-office work with in-person community meetings across Harrison County, Indiana. You will support individuals through Medicaid waivers, crafting plans and coordinating services with providers.

The role requires a human services degree or equivalent experience, valid driver's license, and the ability to travel throughout the county.

Qualifications

• Bachelor's degree in a human services field or related experience.
• Ability to travel within Harrison County and surrounding counties for meetings.
• Pass background screens and maintain a designated home office setup.

Responsibilities

• Educate Individuals about the Medicaid waiver process.
• Create individual support plans based on clients' needs and goals.
• Help access services and supports to meet client goals.
• Facilitate community-based team meetings to review care plans.
