Indiana Professional Management Group (IPMG) is seeking a full-time Case Manager to support Huntington County, IN, with a hybrid work arrangement that includes remote work and travel for community-based meetings several times weekly.

The role focuses on guiding individuals through Medicaid waivers, building individualized plans, and coordinating services with providers to meet goals. Travel within the county is required and applicants must reside in Huntington County.

Qualificações

• A Bachelor’s degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or
• A Bachelor’s degree in an unrelated field and a minimum of one year of direct experience with individuals with intellectual/developmental disabilities, or two years with aging population.
• Valid driver’s license, car insurance, and reliable transportation
• Ability to pass background screens
• Home office with computer, internet, and smartphone

Responsabilidades

• Provide information and education about Medicaid waiver process
• Create an individual support plan based on individuals' strengths, needs, goals, and desires
• Help individuals access services and supports to meet goals
• Facilitate regular community-based team meetings to review plan of care
