Job summary

Indiana Professional Management Group (IPMG) is seeking a Case Manager to serve Harrison County, IN in a full-time hybrid role. You will educate about Medicaid waivers, develop individual support plans, and connect individuals with needed services while coordinating with local partners.

Applicants must reside in Harrison County, Indiana, be able to travel for meetings, and operate a home office with computer, internet, and smartphone.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field; or
• Bachelor's degree in an unrelated field with a minimum of one year direct experience with individuals with developmental disabilities or two years with aging population.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the individuals we serve.
• Help individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.
