Qualifications

• RN license in the state of the office or in a reciprocating state.
• Completion of case management orientation program.
• Minimum of 2 years full-time acute inpatient setting or specialty area.
• Excellent communication skills.
• Basic typing with 35 WPM; prior case management experience is helpful.

Responsibilities

• Develop care plans to meet the patient’s health care needs.
• Utilize clinical nursing experience to advocate for patients.
• Grow professionally in health care knowledge and perspective.
• Assist with orientation and transition to hybrid work after orientation.
