Job summary

IPMG is seeking a Case Manager in a full-time hybrid role serving Greene County, Indiana. The position blends remote work from a home office with travel to attend community-based meetings several times a week, within Greene County and surrounding areas as needed.

Responsibilities include educating individuals about Medicaid waivers, creating tailored support plans, helping clients access necessary services, and coordinating with providers through regular team meetings.

Qualifications

• Bachelor's degree in a human services field or related experience as described.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.
• Designated home office with computer, internet and smartphone.

Responsibilities

• Provide information and education about the Medicaid waiver process.
• Create an Individual support plan based on strengths, needs, goals and desires.
• Help Individuals access services and supports to meet goals.
• Facilitate regular community-based team meetings to review plans and coordinate with providers.

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation or another human services field

Bachelor's degree in an unrelated field with 1 year direct experience or 2 years experience with aging population
