Job summary

Community Mental Health for Central Michigan is seeking a full-time Case Manager to join a collaborative clinical team in a hybrid work setting. The role focuses on coordinating person-centered care for individuals with mental health needs, intellectual/developmental disabilities, and co-occurring disorders, combining health and social supports.

Qualifications

• Bachelor's degree required.
• Motivational Interviewing and Trauma Informed Care training preferred.
• Three years of experience with children and families preferred when providing services to children.

Responsibilities

• Uses guided discovery to help individuals identify presenting problems and build rapport.
• Develops and reviews Person-Centered Plans with interventions to enhance social functioning and link to services.
• Coordinates care with the treatment team, monitors progress, crisis intervention, and safety planning.
