Job summary

CNS Healthcare, based in Detroit, MI, seeks a Case Manager for Wayne County Offices to support population-focused care and integrated health services. You will coordinate care, assess needs, and develop service plans within a trauma-informed framework.

The role offers community-based work with some remote flexibility after training, with generous benefits and PSLF/loan forgiveness possibilities. QMHP eligibility and a human-services degree are required.

Qualifications

• QMHP credential or eligibility documentation.
• Bachelor’s degree in a human services field.
• Preferred Master’s degree in a related field.
• Valid, unrestricted driver’s license and reliable transportation.
• Strong knowledge of mental illness, medications, and interventions.
• Ability to use motivational interviewing and trauma-informed approaches.
• Excellent communication, discretion, and teamwork.

Responsibilities

• Provides community-based case management services across mental and behavioral health.
• Assesses needs, coordinates care, and develops person-centered plans.
• Gathers information and conducts annual intake and substance use assessments.
• Coordinates medication management and works with healthcare providers.
• Facilitates groups and offers transportation assistance as needed.
• Maintains documentation per CNS policies and regulatory standards.
