Qualifications

• Pass background checks and screening required for health programs.

Responsibilities

• Perform functional assessments leading to individual plans and access to services.
• Review plan effectiveness and adjust to maximize client support.
• Assess needs and ensure continuity of care with referrals to resources.
• Maintain daily records and documentation for funding agencies.
• Attend monthly client meetings to monitor services.
