Job summary

Vail Communities is seeking a Mental Health Case Manager in Hopkins, MN to provide direct client support, coordinate case management, and document care plans. The role offers a hybrid work model, joining a team dedicated to helping adults with serious mental health conditions access resources and services in the community.

You will build relationships, facilitate referrals, and advocate for clients while navigating state guidelines and developing individualized support plans.

Qualifications

• Bachelor’s or Master’s in Human Services, Psychology, or Social Work, or qualify as a Mental Health Practitioner.
• Social Work licensure for Social Work candidates.
• At least one year of professional or internship experience in the mental health field.
• Valid Minnesota Driver’s License and access to a reliable vehicle.

Responsibilities

• Provide direct client support and build trusting relationships.
• Coordinate case management and referrals with agencies and professionals.
• Complete required documentation including assessments and plans per Minnesota guidelines.
• Provide crisis intervention and immediate support in crisis situations.
• Regularly meet with clients to monitor progress and adjust care plans.
• Build strong community connections with officials, families, and organizations.
