Job summary

Rural-Resources in Washington seeks an MTP Case Manager to deliver Medicaid MAC, TSOA, FCSP and related Senior and Independent Living Services. You will conduct caregiver-centered assessments, develop and implement service plans, and coordinate supports to promote client independence.

The role requires in-home monitoring, PANs as needed, and collaboration with families, clients, and providers while following WAC and program guidelines.

Qualifications

• Minimum education/experience as per policy: multiple paths to eligibility.
• Specific education routes include high school diploma with extensive social service experience or relevant degrees.

Responsibilities

• Complete initial client/family interviews to assess eligibility.
• Gather data to form psychosocial assessments.
• Facilitate group discussions to design intervention plans.
• Monitor plan adherence and information flow.
• Provide crisis support and in-home client monitoring.
