Job summary

World Relief seeks a Health and Wellness Case Manager to support vulnerable immigrants, providing Intensive Case Management and wellness programming. The hybrid role involves office presence four days a week with one day remote, travel between High Point and Winston-Salem as needed, and collaboration with health providers to deliver trauma-informed, compliant care.

Qualifications

• Experience in case management, health education, or related fields required.
• Experience with refugee or vulnerable populations preferred.
• Fluency in English required; bilingual in listed languages preferred.

Responsibilities

• Oversee referrals, conduct intake assessments, and enroll clients.
• Provide direct Intensive Case Management to develop individualized goal plans.
• Maintain regular communication with clients to monitor progress and needs.
• Guide clients through healthcare, social service, and community systems.
• Maintain accurate documentation and case notes in compliance with guidelines.
• Collaborate with Programs Manager and staff to streamline service delivery.
