Job summary

IPMG is a fully employee-owned organization through its ESOP, and is seeking a Case Manager in a full-time hybrid role serving Tippecanoe County, Indiana. The position requires residency within Tippecanoe County and travel to community-based meetings several times a week.

You will provide information and education on Medicaid waiver processes, develop individual support plans, help clients access needed services, and lead regular team meetings with providers to review plans of care.

Qualifications

• A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or
• A Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population
• A valid driver's license, car insurance, and reliable transportation
• The ability to pass thorough background screens
• Designated home office setting with computer, internet service, and smart phone

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Education
