Job summary

Mosaic is seeking a Targeted Case Manager to support people with developmental disabilities, coordinating services with the person and their network to promote independence and integration. The role emphasizes planning, documentation, and ongoing service coordination.

Typical schedule is 9a-5p, Monday–Friday, with a hybrid work setup between home and community. Requires at least six months of relevant human services experience and a Bachelor's degree or equivalent experience in developmental

Qualifications

• Six months in human services, and either a Bachelor's degree or additional full-time experience in developmental disabilities services
• Six months of full-time experience in human services; degree or equivalent experience to substitute for missing college semesters

Responsibilities

• Manage development, updating, and review of the Person-Centered Support plan with the person and support network
• Monitor the quality of services delivered to individuals served
• Document individual progress and maintain records of persons served
• Conduct and record daily billable activities
• Provide information about types and availability of community services and supports
• Access natural supports and community systems and pursue means to gain access to needed services
• Modify service systems to improve accessibility
• Monitor and follow up to ensure the plan is effectively implemented
• Assist with transition and portability when a person moves
• Comply with KDADS Rules of Conduct for Case Managers Serving People With Developmental Disabilities

Education

Bachelor’s degree or equivalent experience in developmental disabilities services
