Job summary

ACCORD is seeking a dedicated Case Manager to support clients in the Dakota, Hennepin, and Washington County areas. The role includes assessing needs, creating care plans, coordinating services, and monitoring progress to ensure appropriate care.

Hybrid work arrangement with office presence in St. Paul and community/home visits. Full-time, non-exempt, 40 hours weekly. Compensation includes a $23–$25 hourly wage and a $1,500 new hire bonus paid quarterly.

Qualifications

• Bachelor’s Degree in Human Services, Social Work, Psychology, Sociology, or a related field.
• If you hold a Social Work degree, licensed as required by Minnesota Board of Social Work.
• ORBachelor’s degree in any field with at least one year of experience working with individuals with a mental illness or disability.
• Good written and oral communication skills.
• Excellent at working independently and in crisis situations.
• Comfortable with high-volume paperwork.
• Attention to detail and process oriented.
• Able to pass DHS background check and valid driver’s license.

Responsibilities

• Developing a community support plan (goals) for each individual (typical caseload 40-45).
• Assist individuals to access services and document through case notes.
• Inform the person or guardian of suitable service options.
• Meet with individuals at least twice a year to evaluate progress and adjust needs.
• Identify potential providers to guide individuals toward goals.
• Evaluate and monitor services identified in the plan.
• Follow case management rules and MN Health Care Program requirements.
