Job summary

Under supervision, you will mentor staff, manage budgets, and ensure compliance with federal/state guidelines while maintaining strong relationships with families and community partners.

Qualifications

• Education: Graduation from an accredited college with a bachelor’s degree in specified human services fields.
• Substitution: Experience may substitute for education on a year-for-year basis (max 4 years).
• Substitution of Education: Graduate coursework may substitute for the required experience (max 1 year).
• Necessary Special Qualifications: Some positions require qualification under ICF/IDD regulations.

Responsibilities

• Uses resources to determine eligibility and compliance with guidelines.
• Schedules face-to-face visits in home and community to plan services.
• Mentors new employees on departmental processes.
• Estimates annual budget for services and develops individual support plans.
• Creates and maintains plans, documentation, and monthly reviews for service delivery.
• Facilitates planning discussions and negotiates plan changes.
• Provides clear information to team and uses person-centered planning tools.
• Develops cooperative relationships and explains materials to stakeholders.
