Job summary

Tennessee Community Services Agency is seeking a Case Manager to provide case management services for individuals with developmental and intellectual disabilities enrolled in the Katie Beckett Part B Program.

You will conduct meetings, develop ISPs, coordinate services, and maintain accurate documentation while complying with DDA requirements. A Bachelor's degree and valid driver's license are required; ISC certification is a plus, and travel is expected across service areas.

Qualifications

• Bachelor’s degree is required; preferred fields include Social Work, Psychology, Sociology, or related field.
• 1-2 years of related work experience preferred.
• ISC experience and/or certification preferred.
• Knowledge of DDA and Katie Beckett Program a plus.
• Must have valid driver’s license and reliable transportation.
• Must successfully complete background checks ( FBI / TBI ).

Responsibilities

• Meet with individuals, families, and support team.
• Develop ISP based on each individual's needs and goals.
• Submit ISPs to DDA to request services and supports.
• Complete monthly monitoring of authorized services to ensure appropriate support.
• Maintain accurate case documentation and required paperwork.
• Coordinate with individuals, families, providers and others.
