Job summary

State of Tennessee is seeking a crisis stabilization facilitator within the Disability and Aging (DDA) department in Murfreesboro. The role involves on-site and remote consultations, crisis intervention services, and development of individualized plans for assigned caseloads.

Requires a bachelor’s degree in psychology, social or health sciences and at least two years of relevant experience; a master’s degree or internship can substitute for experience.

Qualifications

• Education and experience aligned with psychology, social science, education or health science with at least two years of work with individuals with IDD or mental health disorders.
• Masters substitution permitted for experience on a year-for-year basis up to one year.
• Internship within the Department can substitute for experience.

Responsibilities

• Provides on-site and/or remote consultation and intervention services for individuals with IDD as needed.
• Conducts individualized assessments for referrals and gathers records.
• Collects client information to develop individualized crisis stabilization plans.
• Coordinates external partnerships with third-party agencies and organizations.
• Provides training for caregivers and external entities when necessary.
• Maintains industry knowledge to perform needed interventions and trainings.
• Acts as liaison and advocate for the client throughout service enrollment.
• Facilitates meetings to identify needs and adjust plans.
• Provides 24/7/365 on-call services, including nights and weekends.
