UCHealth in Aurora, CO is seeking a patient outreach coordinator to support care management through outreach, scheduling, referrals, and transitions of care. This full‑time role offers a hybrid schedule with about 50% remote work and 50% onsite.

You will collaborate with care teams to close care gaps, connect patients with community resources, and resolve access barriers, requiring clear communication, organization, and the ability to work in a fast-paced healthcare environment.

Formación

• High School diploma or GED.
• Preferred: 1 year of relevant experience.
• Preferred: Customer service, medical terminology, or care coordination experience.
• Preferred: 1-2 years' experience in healthcare, care coordination, patient navigation, medical office operations, population health, health plan operations
• Preferred: Experience assisting patients with scheduling, referrals, healthcare navigation, community resources, and barriers to accessing care
• Preferred: Experience conducting patient outreach by telephone, electronic communication to support engagement and completion of healthcare needs
• Preferred: Experience supporting care management, population health, chronic disease, preventative care, or quality improvement programs
• Preferred: Healthcare/Medical terminology

Responsabilidades

• Coordinate patient outreach, appointments, referrals, and transitions of care.
• Support care teams with care gap closure, preventive care, and value-based initiatives.
• Act as liaison between patients and clinics to resolve issues and improve access to care.
• Connect patients with community resources to address socioeconomic barriers.
• Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision.
