• RN license valid in the state of practice.
• Bachelor’s degree in Nursing preferred.
• 1 year of clinical nursing and care management experience.

Responsibilities

• Assesses patient needs and develops care plans per NCQA standards.
• Partners with patients to evaluate health needs, lifestyle choices, and resource utilization.
• Educates, coaches, and advocates for patients regarding treatment options, diseases, medications, and resources.
• Coordinates care across the continuum, facilitates transitions, and works to reduce gaps in care and readmissions.
• Within scope of job, demonstrates critical thinking and the ability to work with minimal supervision.
