Job summary

The position requires an Associate degree with 2–4 years of related experience and a health care worker certification or ability to obtain within 18 months. A hybrid work arrangement is available with competitive benefits.

Qualifications

• Associate's degree from a two-year college or university with 2–4 years related experience.
• Experience in care coordination, outreach, or related roles preferred.
• Traditional Health Care Worker Certification or ability to obtain within 18 months of hire.

Responsibilities

• Coordinate care with PCPs, local agencies, and community resources.
• Conduct member outreach and evaluate member needs.
• Attend Interdisciplinary Care Team (ICT) meetings and care coordination meetings.
• Develop member care plans/shared goals in collaboration with the member and Case Managers.
• Maintain confidentiality and HIPAA compliance.
• Provide education, resources, and referrals for members and support networks.
