Job summary

Cascade Comprehensive Care seeks a dedicated care coordination professional to support members’ needs and promote continuity of care. The role focuses on coordinating with PCPs, community resources, and care teams to deliver effective, cost-conscious support.

You will build relationships with members and families, develop care plans, and advocate for optimal outcomes while maintaining HIPAA standards and collaborating with clinical leadership. Hybrid work option available.

Qualifications

• Associate's degree from two-year college or university; and 2 to 4 years related experience and/or training; or equivalent combination of education and experience.

Responsibilities

• Conducts member outreach and evaluation to determine member needs.
• Coordinates with local agencies, PCPs, and community resources to identify and support members in need of care coordination.
• Attends ICT meetings, care coordination meetings, and other meetings as assigned.
• Maintains knowledge base and referral list of local healthcare resources for care coordination including internal and external referrals.
• Manages care coordination and care setting transitions of all assigned members, consulting with Case Managers as needed.
• Develops working relationship with member, family or representative, and care team as member advocate.
• Develops member care plans/shared goals through evaluation of needs in collaboration with member and Case Managers as needed.
• Reviews cases with Clinical Operations management or Chief Medical Officer when necessary.
• Provides education, resources, and referrals for both the member and family/support system.
• Maintains confidentiality and HIPAA compliance.
• Maintains punctual, regular attendance; works collaboratively in a team.
