Job summary

Qualifications

• 2+ years in healthcare navigation, case management, or related roles.
• Fluent in English and Spanish to support diverse member communities.
• Experience with electronic health records and digital databases.

Responsibilities

• Guide members by phone to navigate care options and benefits.
• Coordinate timely care transitions with care managers and clinical staff.
• Research and resolve inquiries on plan coverage, claims, and records.
• Collaborate across internal teams and external providers to ensure high-quality support.
• Record interactions and care activities in electronic health record systems.
