Qualifications

• MSW required or related field with clinical focus.
• Active licensure (DC LICSW) is mandatory.
• 3+ years in care management or community health.

Responsibilities

• Manage a caseload of members with moderate to complex needs.
• Conduct assessments and community-based outreach visits.
• Develop and monitor individualized care coordination plans.
• Coordinate medical, behavioral health, and community services.
• Support discharge planning and transitions of care.
• Educate members and families.
• Document care activities accurately.
