This hybrid position requires a Master’s in Social Work, DC LGSW, and strong communication skills.

Qualifications

• Master’s degree in Social Work is required.
• Active DC LGSW license is required.
• Two or more years of care management or community health experience preferred.
• CCM certification preferred.
• Spanish language skills preferred.
