Job summary

Keystone Community Services in Saint Paul, MN, is seeking a Care Coordinator to assess, plan, facilitate, and advocate for services for older adults and people with disabilities enrolled with Medica Health Plan.

The role emphasizes case management, coordination of waiver programs, and collaboration with healthcare professionals. Hybrid work includes in-office Wednesdays with flexible scheduling for other days.

Qualifications

• Bachelor’s degree in Social Work or related field.
• Two years experience with Minnesota Home and Community Based Waiver Programs or MSC+/MSHO health plan care coordination.
• Experience with seniors or disabled adults in community settings; assess, refer and follow-up resources.
• Strong written and verbal communication; proficient computer/database skills.
• Excellent organizational skills and ability to manage multiple demands.

Responsibilities

• Provide person-centered case management for Medica Health Plan enrollees, locating waivers and community services.
• Complete community or facility assessments to determine level of care and needs.
• Develop and monitor person-centered care plans with active involvement of the individual.
• Coordinate services, document progress, and adjust plans as needed.
• Educate enrollees on Medicare/Medicaid benefits.
• Liaise with community and referral services; maintain records and documentation.
