Job summary

Banner Health is seeking a Care Coordination onboarding and education specialist to support facility and system leadership across our acute care network in the greater Phoenix area. You will onboard new hires, deliver training, audit KPIs, and analyze data to drive improvements in care coordination and patient flow.

This role requires a Bachelor’s degree in a related field, at least two years in acute/post-acute care, and strong communication and problem-solving skills.

Qualifications

• Bachelor’s degree in business administration, health services administration or related field.
• Two years of experience in an acute or post-acute care setting.
• Knowledge of healthcare management.
• Awareness of regulatory/accrediting agency requirements.
• Critical thinking, communication, influence and analytical skills.
• BLS certification is required.

Responsibilities

• Support Care Coordination leadership in onboarding and orientation of new hires.
• Audit and analyze data metrics and KPIs for defined targets.
• Develop and execute internal communications within the organization.
• Collaborate with leadership to improve care coordination across facilities.
