Qualifications

• RN must possess knowledge of case management or utilization review as normally obtained through the completion of a bachelor's degree in case management or health care.
• Requires current Registered Nurse (R.N.) license in state worked.
• Social Worker: Requires a Master's Degree in Social Work and a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) or MSW with licensure within 6 months of hire date.
• An equivalent license applies to states that do not recognize an LMSW; ensure licensure within 6 months of hire.
• Basic Life Support (BLS) certification is required for acute facilities.

Responsibilities

• Manages individual patients across the health care continuum to achieve optimal outcomes.
• Leads process improvement activities for patient populations to improve outcomes.
• Develops and manages the interdisciplinary discharge plan with the care team.
• Educates on case management and managed care concepts within the health care team.
• Maintains knowledge of Medicare, Medicaid and other program benefits for discharge planning.
• Collaborates with physicians, payers, and health care staff for timely information and regulatory needs.
• Possibly supervises other staff and works within established procedures.
