• Evaluates medical necessity and appropriateness of care; acts as liaison for referring physicians and schedules tests/consultations.
• Coordinates disease care with physicians and providers, using an interdisciplinary holistic approach.
• Ensures patient and family understand diagnostic processes, care strategies, and next steps; provides resources.
• Establishes and implements a plan of care with functional, measurable goals; initiates discharge planning.
• Supports patients and families during difficult decision-making periods, including end-of-life care support.
• Contributes to process improvement activities to achieve optimal clinical and satisfaction outcomes.
• Establishes collaboration with physicians, payers, and health care team; communicates timely information to fulfill requirements.
• Works independently within established procedures and consults supervisor on unusual situations.
