Job summary

Humana is seeking an Onsite Care Coordination Nurse in Milwaukee, WI, combining in-person assessments with hybrid coordination and documentation remotely. You will support patients during hospital-to-home transitions, addressing gaps in care and collaborating with providers for timely, safe discharges.

Minimum 3 years acute care experience is required, with a Wisconsin RN license. This role offers a hybrid schedule and requires travel to an onsite facility within reasonable commuting distance.

Qualifications

• Current, unrestricted RN license in Wisconsin.
• Must live within a 30–60 minute drive to Aurora St. Luke's Medical Center.
• Minimum of 3 years acute care nursing experience.
• Experience with discharge planning or care coordination.
• Experience with Microsoft Office and electronic medical records.
• Valid driver’s license, vehicle, and proof of insurance.

Responsibilities

• Conduct comprehensive face-to-face assessments of newly admitted members at acute care facilities.
• Lead discharge planning from admission through post-discharge, including follow-up visits.
• Identify gaps in care and barriers to discharge; develop and implement strategies.
• Collaborate with providers and care teams to ensure timely, coordinated care.
• Provide education on diagnoses, medications, and preventive measures.
• Complete all required documentation accurately and promptly.
• Maintain adherence to infection control and privacy standards.
