Humana is seeking an Onsite Care Coordination Nurse to support members during critical care transitions. You will perform face-to-face assessments, coordinate discharge planning, and collaborate with hospital staff to ensure safe, timely moves from hospital to home.

The role combines in-person care at assigned facilities with remote coordination and documentation, within a flexible hybrid model. A Kentucky RN license and acute care experience are required; BSN is preferred.

Qualifications

• Current, unrestricted RN license in Kentucky.
• Minimum of 3 years recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital/health plan settings.
• Experience with Microsoft Office applications and EMR systems.
• Valid driver’s license and access to personal vehicle.

Responsabilités

• Conduct comprehensive face-to-face assessments of newly admitted members at acute care facilities.
• Educate members and families on discharge planning through post-discharge follow-up.
• Identify gaps in care and barriers to discharge; develop and implement solutions.
• Collaborate with providers, case managers and healthcare professionals for timely, coordinated discharge planning.
• Provide education on diagnoses, medications, and preventive measures to promote wellness and reduce readmissions.
• Complete all required documentation accurately and promptly.
• Maintain infection control, safety, and PHI protection standards.
