Job summary

Humana is seeking an Onsite Care Coordination Nurse to support members during critical transitions from hospital to home. You will perform in-person assessments, coordinate discharge planning, and collaborate with care teams in a hybrid role combining facility work with remote documentation.

Qualified candidates are Texas-licensed RNs with 3+ years in acute care, experience in discharge planning, and proficiency in MS Office and EMR systems. A BSN is preferred and travel is incidental.

Qualifications

• Current, unrestricted Texas RN license.
• Minimum 3 years recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Proficiency in MS Office and EMR systems.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Conduct face-to-face assessments for newly admitted members at acute care facilities.
• Lead discharge planning from admission through post-discharge.
• Identify gaps in care and coordinate with providers and care teams.
• Educate members and families on diagnoses, medications, and preventive measures.
• Ensure timely, accurate documentation and maintain privacy standards.
