Qualifications

• Current, unrestricted RN license in Wisconsin.
• Located within a 30-60 minute driving distance to Aurora St. Luke's Medical Center, Milwaukee, WI.
• Minimum of 3 years acute care nursing experience (medical-surgical, critical care, etc.).
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Experience using Microsoft Office and electronic medical records.

Responsibilities

• Conduct face-to-face assessments for newly admitted members at acute care facilities to evaluate needs and coordinate care.
• Support discharge planning from admission through post-discharge and follow-up visits with PCPs.
• Identify gaps in care, safety concerns, and barriers to discharge; develop and implement solutions.
• Collaborate with providers and care teams to ensure timely, coordinated care and discharge planning.
• Educate members on diagnoses, medications, and preventive measures to reduce readmission risk.
• Complete all required documentation accurately and promptly.
