Job summary

AuthoraCare Collective in Greensboro is seeking a PRN CAP Case Manager (Social Worker) to manage the plan of care for CAP clients in a hybrid role. The position emphasizes assessment, goal setting, and regular client contact with quarterly in-person visits.

Responsibilities include coordinating services, collaborating with RN case managers, and advocating for patient/caregiver needs while adhering to NC Medicaid requirements and infection control policies.

Qualifications

• Graduate of an accredited School of Social Work.
• One year of experience in social work case management required.
• Bachelor's degree in social work required.
• Case management accreditation or certification preferred by agency.
• Valid State-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.

• Directs and coordinates all services provided through CAP to patients in accordance with the patient's individualized care plan.
• Utilizes case management process in planning and prioritizing patient care needs, including assessment and identification of patient/caregiver problems; identification and implementation of interventions; and evaluation of outcomes.
• Partners with the assigned RN case manager to complete annual recertification process.
• Establishes relationship with patient/caregiver to adequately assess ongoing needs.
• Recognizes influence of culture, ethics, and beliefs on decision making.
• Makes home visits in accordance with NC Medicaid requirements.
• Offer telephonic support to CAP clients as needed.
• Communicates changes to the plan of care to patient/family and any internal/external stakeholders involved.
• Complies with infection control and exposure control policies.
• Maintains effective relationships and partnerships with community healthcare providers and external stakeholders impacting the CAPC population.
