Job summary

Titanium Healthcare is seeking a Lead Care Manager to oversee case management for ECM program members in the San Diego area. The role blends remote work with in-person member visits, requiring travel 70% of the time.

You will develop care plans, coordinate with healthcare providers, and advocate for patients while maintaining thorough documentation. Ideal candidates have at least 1 year in healthcare or care coordination, a valid driver’s license, and a current BLS certification.

Qualifications

• Diploma or GED required; associate degree preferred.
• Minimum 1+ year of experience in healthcare, social services, or care coordination.
• Current and valid Driver’s License with a clean driving record.
• Reliable transportation and proof of auto insurance.
• Current BLS certification from the American Heart Association on start date.
• Distraction-free home workspace with secure internet.

Responsibilities

• Manage an assigned caseload of adult and pediatric members.
• Maintain caseload of up to 50 members with monthly in-person visits.
• Conduct comprehensive assessments of physical, emotional, and social needs.
• Develop individualized care plans based on assessments and medical history.
• Coordinate communication between doctors, social workers, therapists, and care team.
• Monitor progress and update care plans to comply with policy.
• Advocate for patients navigating the healthcare system.
• Provide education to members and families on health topics.
