Job summary

Vitality Hospice & Palliative Care seeks a compassionate Bereavement Coordinator for a hybrid role combining remote and in-person visits. You will develop bereavement services, provide grief counseling, and coordinate programs for families navigating loss.

The role emphasizes collaboration with nurses, social workers, and chaplains, with travel to support patients locally and optional evening/weekend on-call.

Qualifications

• Bachelor's degree or specialized bereavement counseling certification required; Master's preferred.
• Experience providing grief counseling or bereavement support for patients and families.
• Hospice or palliative care experience is preferred.

Responsibilities

• Provide pre-bereavement and grief counseling to patients, families, and caregivers.
• Assess emotional and bereavement needs and develop support plans.
• Offer ongoing bereavement support for a minimum of 13 months after loss.
• Coordinate individual counseling, grief support groups, and programs.
• Plan memorial services and remembrance events.
• Identify at-risk individuals and refer to community mental health resources.
